Family Survey

Hello Central Academy families,

Des Moines Public Schools is asking families to provide us with feedback on your perception of the experiences, environment, barriers, and safety for your children at their schools.  The results of this survey provide us with critical information to better serve our families. The information from this survey is gathered by a third-party vendor and is completely anonymous to both the vendor and DMPS. You will not be asked to provide any personally identifiable information.

By default, the survey will ask about your primary DMPS enrollment (your “home school”).  Then, at the end, you will see a prompt to submit responses for another building such as Central Academy or Central Campus.  Please complete the survey once for each building in which you have children enrolled as students.
